Hospice at Home
Rowcroft Hospice at Home offers a 24 hours a day, 7 days a week palliative care service within the South Devon area, providing support and advice to patients and their carers, and enabling patients requiring end of life care to return to, or to remain in, their own homes.
The Team
Our team is led by Registered Nurses with specialist palliative care experience. We work closely with GPs, District Nurses and the Rowcroft Specialist Care Community Team, as well as other professionals involved in providing care for patients and their families. Our Senior Health Care Assistants can provide up to 24/7 hands-on care and support to patients and their families. They help to support the patients choice to be in their own home in the last few days of their life.
How our service works
• Patients can be referred by a member of the Rowcroft Community Team, GP, District Nurse, Care Home Manager or members of the out of hours healthcare teams.
• Referrals can be taken 24 hours a day, 7 days a week.
• We aim to respond to urgent referrals within 24
hours.
• We act as a resource for GPs, District Nurses and other agencies with regards to all aspects of end of life care including pain and symptom control.
• Advice can be given about other services that can also support you at this time.
• Your GP and District Nurse will remain your primary care givers.
