Care at home
A BBC documentary, ‘How to have a good death’, revealed that 67 per
cent of people wish to die at home, but only 20 per cent do so.
Dying
at home may sound like a simple wish, but it takes a lot to make it
happen.
Community Team
The aim of community specialist palliative care is to enable the
patient to remain living at home enjoying the best quality of life for
as long as possible. In line with the hospice’s other services, the
team responsible, which includes a social worker, medical consultant,
nurses and occupational therapist, and offers multi-disciplinary,
holistic care to the patient and their family.
The team also provides specialist knowledge, skills and
experience to general practitioners (GP’s) and district nurses, social
services, community hospitals, residential homes and nursing homes. The
aim is to ensure the highest quality of care for all people with
life-limiting conditions in South Devon.
Hospice at home
The aim of hospice at home is to support patients and families at
home during the last few months of life. Rowcroft continues to fund
this pilot service, established in 2001. Rowcroft’s hospice at home
nurses work alongside district nurses, GP’s, Marie Curie nurses and
social services to deliver this service.
